Indications:
1. Edema disease -- Excrete excess sodium and water from the body, reduce extracellular fluid volume, and eliminate edema. Common cases include congestive heart failure, cirrhotic ascites, nephrotic syndrome, acute and chronic nephritis edema, early stage of chronic renal failure, and sodium and water retention due to adrenocorticosteroid and estrogen therapy.
2. Hypertension -- Can be used alone or in combination with other antihypertensive drugs, mainly for the treatment of essential hypertension.
3. Central or renal diabetes insipidus.
Nephrolithiasis - mainly used to prevent calcium-containing stones.
Usage And Dosage:
1. Adults often take orally.
(1)For the treatment of edema diseases, 25 ~ 50mg each time (1-2 tablets each time), 1 ~ 2 times a day, or every other day, or even take 3~5 days a week.
(2)For the treatment of hypertension, 25 ~ 100mg per day (1-4 tablets per day), divided into 1 ~ 2 times, and adjust the dose according to the antihypertensive effect.
2. Children often take orally.
According to body weight 1 ~ 2mg/kg or body surface area 30 ~ 60mg/m2, divided 1 ~ 2 times a day, and the dose was adjusted according to the curative effect. Up to 3mg/kg bw/d for infants younger than 6 months.
(1) cross-allergy: cross-reaction with sulfonamides, furosemide, bumetanib, carbonic anhydrase inhibitors.
(2) Interference to diagnosis: decreased glucose tolerance, increased concentration of blood glucose, urine glucose, blood bilirubin, blood calcium, blood uric acid, blood cholesterol, triglyceride and low density lipoprotein, decreased concentration of blood magnesium, potassium, sodium and urinary calcium.
(3) Use cautiously in the following cases:
Interactions:
(1) Adrenocorticosteroid, adrenocorticotropic hormone, estrogen, amphotericin B (intravenous), can reduce the diuretic effect of this drug, increase the chance of electrolyte disorders, especially hypokalemia.
(2) Non-steroidal anti-inflammatory and analgesic drugs, especially indomethacin, can reduce the diuretic effect of this drug, which is related to the inhibition of prostaglandin synthesis by the former.
(3) When combined with sympathomimetic amines, the diuretic effect was weakened.
(4) Corolenide (cholestyramine) can reduce the absorption of this drug in the gastrointestinal tract, so it should be taken 1 hour before or 4 hours after oral corolenide.
(5) When combined with dopamine, the diuretic effect is enhanced.
(6) When combined with antihypertensive drugs, the diuretic antihypertensive effect was strengthened.
(7) When combined with anti-gout drugs, the dosage of the latter should be adjusted.
(8) The weakened effect of anticoagulants is mainly due to the decrease of plasma volume and the increase of coagulation factors in blood after diuresis, and the improvement of liver blood supply and the increase of synthetic coagulation factors due to diuresis.
(9) Reduce the effect of hypoglycemic drugs
(10) When digitalis and amiodarone are used together with this drug, side effects caused by hypokalemia should be carefully prevented.
(11) with lithium preparation, because the drug can reduce the kidney to lithium clearance, increase the kidney toxicity of lithium.
(12) The conversion of urotropine into formaldehyde was inhibited when it was used together with this drug, and the curative effect was reduced.
(13) Enhancing the effect of non-depolarizing muscle relaxants was related to the decrease of blood potassium.
(14) When combined with sodium bicarbonate, the chance of hypochloric alkali poisoning is increased.
